Thank you for your interest in joining the California Telehealth Network.

Please follow the steps below to initiate the membership process: 

1. Complete and return the Letter of Agency (LOA). The LOA will allow the CTN to act on your behalf in matters with our funding agency, the Universal Services Administration Company (USAC) an administrative branch of the Federal Communications Commission (FCC).

2. Complete and return the Participant Funding Eligibility Checklist.  The Eligibility Checklist will allow CTN and USAC to verify the site’s eligibility to participate in the program. 

3. If you are a Mental Health Organization, please completeand return the Mental Health Organization Checklist as well.

Once complete, please sign and mail original copies of all documents to:

Next Steps
 

  • Upon receipt of the signed, original documents, CTN will file the appropriate paperwork with USAC on your behalf. 
  • When CTN is notified by USAC that you are qualified to participate, CTN will contact you with the official membership agreement
  • You will be required to sign and return the membership agreement
  • After that agreement is executed, your circuit(s) will be ordered through AT&T!
