Steps for connecting to the network
Contact Us: Not sure where to begin? Please contact CTN with your interest in learning more about how CTN can assist you in joining the network.
Needs Assessment Form: Complete and submit the Needs Assessment form if your organization is interested in joining CTN, or are current members who would like to continue to receive subsidized broadband services.
Letter of Agency: Complete the Letter of Agency (LOA) form if your organization is:
1) Interested in receiving reduced rate broadband through the Healthcare Connect Fund, and
2) Considered a Healthcare Connect Fund eligible provider: FQHC, Critical Access Hospital, nonprofit rural and community health clinic, Native American health facility, county or municipal health facility, or
3) Current members who would like to continue to receive subsidized broadband services.
CTN Participation Fees are billed annually and calculated based on subsidy amounts received. There is also a floor and ceiling to ensure ROI.
